Withdraw/Refund Policy:  There will be a 10% withdrawal refund fee assessed for all withdrawal refund requests.  A 50% withdrawal refund fee will be assessed for withdrawal refunds requested within 14 days of the program start date.  There will not be any refunds issued after the start of any program except for relocation out of the City of Regina and surrounding areas within 50 kilometers or for medical reasons confirmed by a doctor’s note.  If it is a medical or relocation withdrawal, a prorated refund less the $15.00 withdrawal refund fee will be assessed. The supporting doctor’s note must be received by the Recreation & Athletic Services Office within 10 days of notification of the medical withdrawal.  If the medical note is not received, the refund will not be issued.  If your child or ward is asked to leave the program by the University of Regina, a prorated refund will be provided.

Refunds will be issued within 14 days of withdrawal. If it is a medical withdrawal, refunds will be issued no later than 14 days after the doctor’s note is received.   To withdraw from a program, please contact the Recreation & Athletic Services office in person or by calling (306)585-4371.

Program Cancellations and Closures:  Please note that programs may be cancelled for any reason by the University of Regina either for a specific class date or in its entirety.  If a program is cancelled in its entirety, a pro-rated refund will be provided.  If a specific class date within the program is cancelled, a make-up class may be provided. Please note that this may not always be possible. 

The University of Regina is closed on all statutory holidays as well as any University of Regina closures for maintenance or other reasons.  This includes the period from December 25 to January 1 inclusive.

Transfer Policy: Transfers are allowed within the same program area up to one day prior to the start date of the program, space permitting. An example of this is if the original registration is in Summer Sports School, the registrant may transfer into another Summer Sports School program but not into a Cougar Camp.  Registrants may transfer with no charge up to 3 days after they registered in the program.  A $10.00 fee will be assessed for all transfers after the three day grace period.  No transfers are allowed after the start date of the program.

EYES strives to mitigate risks in all our activities by working closely with the U of R’s Health, Safety & Environment, Risk Management, and Human Resource units and all activities are reviewed by multiple individuals in a three tired safety and security review. EYES instructors are provided First Aid and U of R Safety training as safety is a core component of our programming. However, there are inherent risks that are associated in participating in the program that may result in illness, personal injury or damage.

Privacy Clause:  I understand that the University of Regina collects and creates information about EYES campers for purposes of admission, registration, recruitment, safety, promotion, and the administration of the University and its programs and services. Some of this information may be reported as required by federal or provincial authority. By enrolling in EYES at the University of Regina, I consent to the collection, use, and disclosure of my own and my child’s personal information as described above. I understand that personal and/or medical information may be stored in a University of Regina database and/or in paper form in a physical location at the University of Regina.

Photography/Use of Image Clause:  I give the University of Regina, EYES, its sponsors and Actua permission to use my child’s photographs and likeness in any program informational or marketing material in any medium, and/or to televise my child’s participation in program activities for the purpose of promotion, fundraising, marketing, documentation and public display.

Medical Clause In case of emergency regarding my Child, I understand every effort will be made to contact me.  In the event that I cannot be reached in an emergency situation, I hereby give permission to University staff, licensed emergency and health care personnel to provide treatment/services they deem necessary with respect to my Child.   In the event of medication, medical advice, treatment and/or equipment are required; I agree to accept financial responsibility for fees in excess of provincial and or private medical insurance.  I agree that all medical information provided by the Guardian/Parent to the University may be disclosed to such emergency and health care personnel.  In the event of illness, accident, emergency, or any other circumstance requiring medical treatment, such treatment may be procured for the Participant without legal or financial obligation to the University.

I understand that it is my responsibility to advise the University of any Medical Information with respect to my Child that the University staff should be aware of. I understand and agree that the University will disclose this medical information as required to appropriate University staff. I understand that the University staff may not be medical professionals.  I agree that my Child is medically fit to participate in the Program.   I understand that medical and/or personal information will be stored in a University database and/or in paper form at a physical location at the University.

Online programs offered through the University of Regina (the “University”) does not provide child or ward supervision.  It is the sole responsibility of the parent or guardian of the child or ward to provide supervision during the online programs.  This waiver, in its entirety, applies to all programming offered by the University of Regina (the “University”), including online programs.

Three Strike Policy:  EYES believes in providing a safe and inclusive space for both all children and staff that are involved in the EYES programming, because of this, EYES has a strict no-bullying policy. To ensure this, EYES implements a 3-strike policy. Please note that the strikes are dependent on the severity of the action, determined by the instructor.  This means a youth may not receive a strike 1 before a higher strike is issued. 

Strike 1: Child is given a verbal warning regarding the unwelcome behavior.

Strike 2: Child is given a second verbal warning, and the parent or guardian of the child is informed via phone call at the time of the incident.

Strike 3: Parent is called and the child removed from EYES. No refund is issued.

Strikes are issued at the discretion of the staff. All staff are trained formally on classroom management and classroom safety.

Examples of common unwelcome behaviors are (but are not limited too): name calling, pushing, not listening to instructions, compromising the safety of other children (such as leaning back on chairs or throwing supplies), swearing, or stealing. At the beginning of all EYES sessions, staff review rules and expectations with all children involved in the program. Please review the rules before programming with your child! Rules are posted in all activity rooms.

Informed Consent:  I hereby give consent for my child’s participation in the EYES and related activities on and off campus. I understand that EYES is a program designed to encourage scientific interest and will involve hands-on activities and laboratory experiences. I agree that neither EYES, Actua, nor the University of Regina will be held liable for any injury to my child, or loss or damage to my child’s personal property. In consideration of my child being allowed to participate in EYES, I, the parent/guardian of the child, on my own behalf and on behalf of my child, waive all present and future claims against the EYES, Actua, the University of Regina, and its directors, Board of Governors, employees, officers, servants, representatives, insurers and agents (and their respective successors and assigns) (collectively, the Releasees) and hereby release the Releasees from and against all liabilities, claims, actions, demands, costs and expenses relating to injury, illness, death, loss, damage to person or property or loss of property, foreseen or unforeseen, howsoever caused (including negligence of any one or more of the Releasees), arising out of or in connection with my child’s participation in EYES.  I, on my own behalf and on behalf of my child, also agree to indemnify the Releasees for, on account of or by reason of any claim advanced against any of them, or any loss or damage sustained by them, arising out of my child’s participation in EYES.

I have read, understood and agree to the terms of this agreement. I hereby execute this release, waiver and indemnification on my own behalf, and on behalf of our respective heirs, successors, representatives and assigns.
